Review a finished piece of work and say what failed, why and what to change.
What you learn
- Build a deviation table with achieved minus specified, in its unit and, when the quantity allows it, as a percentage, and a verdict according to the direction of each requirement.
- Decide whether a deviation belongs to the product, to the test, or cannot be decided, from the test conditions and from how much the measurement can vary.
- Tell a verification finding from a validation finding and name an impact that the specification did not ask about.
- Chain together the direct cause, contributing factors, intermediate cause and root cause, and check each one with the question “if it had been eliminated or modified, would the outcome have been prevented?”.
- Write a close-out report with deviations, causes without blame, and actions with an owner, a deadline and a check with a number.
